INEQUIDADES TERRITORIALES Y DETERMINANTES ESTRUCTURALES DE LA SALUD EN TRES MUNICIPIOS COSTEROS DEL DEPARTAMENTO DEL CHOCÓ COLOMBIA
Las inequidades en salud en territorios aislados constituyen un desafío persistente para la garantía del derecho a la salud en América Latina, especialmente en contextos marcados por exclusión histórica y rezago institucional. Este estudio tuvo como objetivo analizar las inequidades territoriales en salud y su relación con los determinantes estructurales en Bahía Solano, Nuquí y Bajo Baudó. Se adoptó un diseño observacional, descriptivo y de corte transversal, con aplicación de cuestionario estructurado a 383 participantes entre junio y julio de 2019, complementado con actualización contextual en 2022. Los resultados evidencian que, aunque predomina la afiliación al régimen subsidiado, el acceso efectivo permanece limitado por barreras geográficas y baja capacidad resolutiva local. La atención especializada depende de remisiones externas, con tiempos de espera entre 30 y 60 días, lo que genera insatisfacción y desconfianza en el sistema. Persisten enfermedades transmisibles asociadas a determinantes ambientales y deficiencias en saneamiento básico, lo que refuerza las inequidades estructurales. En conjunto, los hallazgos se alinean con la evidencia internacional sobre penalización geográfica y con los debates en torno a racismo estructural y justicia social. Reducir estas brechas exige políticas diferenciadas y territorializadas que integren salud, infraestructura básica y conectividad, incorporando la participación comunitaria como componente central en la construcción de estrategias sanitarias.

Abstract:
Health inequalities in isolated areas are a persistent challenge to guaranteeing the right to health in Latin America, especially in contexts marked by historical exclusion and institutional backwardness. This study aimed to analyze territorial health inequalities and their relationship with structural determinants in Bahía Solano, Nuquí, and Bajo Baudó. An observational, descriptive, cross-sectional design was adopted, with a structured questionnaire administered to 383 participants between June and July 2019, supplemented with a contextual update in 2022. The results show that, although affiliation with the subsidized regime predominates, effective access remains limited by geographical barriers and low local response capacity. Specialized care depends on external referrals, with waiting times of between 30 and 60 days, which generates dissatisfaction and mistrust in the system. Communicable diseases associated with environmental determinants and deficiencies in basic sanitation persist, reinforcing structural inequalities. Overall, the findings are consistent with international evidence on geographic penalization and with debates surrounding structural racism and social justice. Reducing these gaps requires differentiated and territorialized policies that integrate health, basic infrastructure, and connectivity, incorporating community participation as a central component in the development of health strategies.
